Housing Stabilization Services

A housing program designed to assist individuals with disabilities and seniors in securing and maintaining stable housing. This service is classified as a Medical Assistance (Medicaid) benefit under the state plan and does not require eligibility for a waiver program. Through Housing Stabilization Services, individuals receive comprehensive support in planning and locating suitable housing options, alongside ongoing support to ensure the continued stability of their living arrangements. Additionally, our team works diligently to identify and address any barriers to housing stability, providing tailored solutions to overcome challenges and promote lasting security.

Eligibility:

  • Be on Medical Assistance (MA)
  • Be 18 years old or older
  • Have a documented disability or disabling condition
  • Be assessed to require assistance with at least one of the following areas resulting from the presence of a disability or a long-term or indefinite condition
  • Be experiencing housing instability, evidenced by one of the following risk factors
  • At risk of institutionalization – meets an institutional level of care/eligible for the following waivers

Person-Centered Planning

Person-centered planning is a process for planning for the life that the person wants for themselves. It involves working closely with the person and their support network to help develop a plan that places the individual at the center. This approach empowers people to plan their life, find their voice, and work toward reaching their goals. In this planning process, we apply person centered thinking and person-centered principles to ensure that the individual remains the focal point of all components.

Eligibility:
This service falls under the Person-Centered, Informed Choice and Transition Protocol, which guides approval. Regardless of their age or the program they participate in, the protocol applies to all children and adults who are eligible to receive support planning. Talk to your Case Manager, School Social Worker, or Care Coordinator if you meet the following criteria and would like to have a formal person-centered plan developed for you. Specifically, this includes the following people who use long-term support and services:

  • Children and adults with disabilities
  • Children and adults with mental illness
  • People older than 65 years old
  • Anyone from the above groups who is also transitioning from one residence to another.

CDCS Support Planning

CDCS support planning services help people with disabilities create a plan for their care that is tailored to their unique needs and goals. Support planners provide information about different care options and work with the individual to create a person-centered plan (CSP). The CSP includes all the necessary components and is based on the person’s assessed needs. Once the plan is developed, the support planner submits it to the lead agency for approval.

The plan is then implemented, monitored, and evaluated on an ongoing basis, with the option to make changes as needed. The support planner also helps the person recruit, hire, and train workers, and provides information about community resources related to the plan. Overall, CDCS support planning services aim to empower individuals with disabilities to make informed decisions about their care and live as independently as possible.
